PROCESS FOR ELECTRONIC/PAPER WAIVERS (July 2020)
Please follow the instructions below for obtaining signatures and processing waivers:
- For players who reside within ISD 721, but wish to play at a different hockey association than NPHA, the parents can initiate the waiver process by completing the D6 Waiver Request form and sending it to the NPHA Registrar.
- Registrar completes electronic/paper waiver, noting restrictions (if any) and sends on to the President for signature.
- The NPHA Registrar then sends the waiver to the receiving association's Registrar for review and signature from the receiving association President.
- Electronic waivers can only be used within District 6 and with District 3.
**All other waivers must be done on the 4-part paper waiver forms.**
- Waivers that are releasing From District 3, Scott Dornfeld needs to sign and stamp s.dornfeld6@gmail.com.
- Waivers that are both releasing and receiving within District 6 Associations, Chris Evans needs to sign and stamp mevans3656@q.com. Chris will then send an approved copy back to registrar for records/rostering.
Other important notes:
*Players will not be allowed on ice unless their COMPLETED and SIGNED waiver is on file.
*Registrars are responsible for making sure addresses are within their affiliate boundaries and identifying all players that need waivers. Players may not tryout or be rostered without a completed waiver.
* Any player on your roster must be eligible. If questions come up the association must be able to provide a completed waiver where applicable. If there is an ineligible player on your roster your team may face disqualification.
*All stamps need to be in color.
